Population Health

Collaborative projects promoting  health and wellness for all ages – with a strong focus on disease prevention and healthy lifestyles. This includes connecting vulnerable populations to programs and services that support physical, mental, and oral health through access to preventive care, exercise, nutritious food, health education, and initiatives that foster long-term well-being and reduce health disparities by addressing social determinants of health.

1889 Foundation has established the Center for Population Health in Johnstown. The Center will identify, quantify and develop strategies for improving the health of the citizens of Cambria and Somerset Counties.

Physical Health & Wellness

Projects in this category provide safe opportunities to engage in exercise, disease prevention and screenings, and access to all forms of healthcare.

Mental & Behavioral Health

These projects provide an array of mental and behavioral health services for all ages, including substance use prevention and treatment.

Social Determinants of Health

The social determinants of health are non-medical factors that influence health outcomes, including food and nutrition, housing, transportation, employment, and social needs.
